Description

You will manage a virtual panel of members as their primary care provider, delivering care via phone, messaging, and video. You'll collaborate with physicians and behavioral health specialists while using new tools to shape the future of virtual primary care.

Requirements

• DNP, FNP, ENP, or PA from accredited program
• Board Certification (NCCPA or AANP or ANCC)
• Spanish language fluency (read, write, speak)
• 3+ years in-person primary care experience
• Licensed in GA, FL, or TX

Responsibilities

• Provide virtual primary care for a panel of patients (18+) via phone, messaging, and video
• Manage chronic conditions (DM, HTN, COPD, CAD) and provide continuity of care
• Collaborate with physicians on complex cases and escalate as needed
• Follow up on labs, imaging, and ensure closed-loop communication with patients and providers
• Participate in collaborative care with virtual behavioral health specialists
