You'll conduct in-home health risk assessments for members in Kent County, MI. Your work will directly impact care management and treatment outcomes through comprehensive evaluations. This role offers flexible scheduling and paid training.

🎯 What You'll Do

• Visit members for in-home health risk assessments.
• Evaluate past medical history and review systems.
• Perform medication reconciliation and comprehensive physical exams.
• Formulate diagnosis list for care management.

📋 Requirements

• Valid unencumbered Michigan NP license.
• 3 years of patient care experience required.
• Proficiency with EMR systems preferred.
