Description

You will provide skilled nursing care to patients in their homes, helping them recover from illness, injury, or surgery. You'll manage cases, develop care plans, and perform assessments, medication management, wound care, and more. This is a per diem weekend role offering flexibility and 1:1 patient care.

Requirements

• Active and current RN license
• One year clinical experience preferred
• Ability to travel locally to cases

Responsibilities

• Visit patients in their homes for 1:1 direct care
• Case manage and develop care plans
• Deliver skilled care including assessments, medication management, wound care
• Progress patients from discharge to meeting goals
• Provide education to patient and family
