💼 About This Role

You'll provide virtual primary care to Medicare patients with chronic, complex needs as part of a multidisciplinary team. You will manage conditions like hypertension and diabetes through telemedicine, closing care gaps and improving outcomes. This role leverages AI and automation to reduce administrative burden.

🎯 What You'll Do

• Provide virtual care and clinical assessments for Medicare patients across multiple markets
• Support initial patient assessments, care plan direction, and ongoing clinical oversight
• Manage chronic conditions, acute concerns, medication-related questions, and routine follow-ups
• Partner with RNs, Community Health Workers, and clinical leadership to close care gaps

📋 Requirements

• 2+ active NP licenses in Pair Team's launch states including California, Nevada, Pennsylvania, etc.
• Board certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
• 2+ years of clinical experience as a Nurse Practitioner
• Experience caring for Medicare, geriatric, or medically complex adult populations
