Description

You will provide compassionate home health care to adult and geriatric clients on a per visit basis in Baker County, FL. Your work ensures clients can remain at home while receiving the medical care they need.

Requirements

• Current Registered Nurse license in Florida
• Minimum one year of recent clinical (medical/surgical) experience
• Graduation from an accredited nursing program
• Ability to travel to client cases as assigned
• Solid computer skills; EMR experience preferred

Responsibilities

• Follow designated care plans and make home visits to clients in assigned geographic territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to Clinical or Client Services Manager
• Perform client assessments and case management
• Accurately document observations, interventions, and evaluations using a touch pad tablet
