💼 About This Role

You'll provide home health visits to adult and geriatric clients throughout Delaware County as part of a multi-disciplinary care team. You'll develop and implement individualized care plans to promote independent living.

🎯 What You'll Do

• Perform diagnostic tests and measurements of mobility, strength, and balance
• Develop and implement individualized care plans with therapeutic exercises
• Educate clients and family members on rehabilitative care
• Document patient care using electronic medical records on a tablet

📋 Requirements

• Current Pennsylvania Physical Therapist license
• Minimum one year of recent experience as a Physical Therapist
• Graduation from an APTA-approved program
