Description

You will provide home health visits as a Medical Social Worker, conducting clinical evaluations and connecting clients with community resources to support their well-being and independence at home.

Requirements

• Master's Degree in Social Work
• Licensed Clinical Social Worker (LCSW) in Colorado
• One year of experience as a Licensed Social Worker
• Prior home health experience preferred

Responsibilities

• Conduct home visits to provide clinical social evaluations
• Plan interventions based on evaluation findings
• Refer clients to appropriate community resources
• Advocate for clients and families to navigate community systems
• Participate in multidisciplinary care plan development
