You'll guide seniors through the eligibility process and manage initial home visits as part of the Outreach Enrollment team. Your work directly helps seniors access community-based medical care and stay in their homes.

🎯 What You'll Do

• Drive enrollment activities and manage initial home visits
• Develop relationships with prospective participants and families
• Complete documentation in CRM software and Salesforce
• Cross-train and provide coverage for Outreach Enrollment team roles

📋 Requirements

• Bachelor's degree
• Minimum 2 years in sales and marketing role
• At least 1 year in healthcare or senior care setting
• Reliable transportation

✨ Nice to Have

• Experience with Microsoft Excel, Word, Outlook
• Familiarity with Salesforce or similar CRM
