💼 About This Role

You'll provide after-hours hospice nursing care to patients in their homes, managing pain and symptoms while supporting families. This role rotates one week on (weeknights + continuous weekend) and one week off, offering a unique schedule.

🎯 What You'll Do

• Complete initial and comprehensive patient assessments for hospice needs.
• Administer medications and treatments as prescribed by physicians.
• Coordinate patient care after hours with case managers and physicians.
• Maintain accurate clinical notes and communicate patient changes.

📋 Requirements

• Active RN license in Indiana in good standing.
• Associate's or Bachelor's degree in nursing.
• Current CPR certification.
• 2-5 years of hospice or palliative care experience.

• CHAP and Medicare Hospice Certification experience.
• Knowledge of interdisciplinary hospice care.
