Description

You will perform outpatient therapy evaluations and develop care plans for patients in senior living communities or group homes, working closely with a multi-disciplinary team to maximize functional independence. You'll use current research-based approaches and an outpatient EMR platform to document and deliver individualized care.

Requirements

• Current NJ Occupational Therapist license
• Minimum 1 year recent experience as Occupational Therapist
• Graduation from accredited occupational therapy program
• Ability to work independently and manage time effectively
• Solid computer skills; EMR experience preferred

Responsibilities

• Perform outpatient therapy evaluations and develop care plans
• Deliver research-based occupational therapy to maximize functional independence
• Collaborate with home health therapy partners to transition care
• Educate clients, family, and caregivers on care plans
• Complete evaluations, treatments, and discharge planning using EMR
