Description

You will provide in-home physical therapy to adult and geriatric patients in South San Antonio and Pleasanton, helping them restore strength, balance, and mobility so they can safely navigate their homes and communities. You'll evaluate patients, develop treatment plans, implement exercise programs, and educate families on recovery and prevention.

Requirements

• Active physical therapy (PT) license in good standing
• BLS CPR certification
• Reliable transportation to see patients in their homes
• Basic comfort with technology (email, texting, EMR systems)

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients
• Evaluate patient mobility, strength, and function to develop individualized treatment goals
• Implement effective rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate and timely home health care documentation per Medicare and agency standards
