Description

You will join a mobile primary care team delivering high-quality, relationship-based care to seniors in their homes and communities. You'll work autonomously with team support, focusing on chronic condition management, dementia-informed care, and coordination with palliative, psych, and neurology specialists.

Requirements

• Active, unencumbered NP or PA license in the state of practice
• Minimum 1-2 years clinical experience (geriatrics or primary care preferred)
• Compassionate, patient-centered approach and strong clinical judgment
• Valid driver's license and reliable transportation

Responsibilities

• Deliver personalized high-quality care to seniors in homes, assisted living, hospice, and nursing facilities
• Conduct comprehensive assessments and develop tailored care plans
• Manage chronic conditions with skill and empathy
• Collaborate with palliative, psych, and neuro teams for coordinated whole-person care
• Build trusted relationships with residents, caregivers, and facility partners
