Description

You will directly outreach and engage individuals with serious mental illness, substance use disorder, or high medical needs, acting as a Lead Case Manager to connect them to whole-person care. This role combines remote work with 2-3 monthly on-site visits in Stockton, California, supporting a mission-driven team that reduces emergency visits and hospital admissions.

Requirements

• Bilingual in Spanish
• Experience working with underserved populations
• Strong communication and relationship-building skills
• Ability to work remotely with occasional on-site visits

Responsibilities

• Outreach and engage individuals with Serious Mental Illness/Substance Use Disorder
