You'll join a multi-disciplinary team providing rehabilitative care to adult and geriatric clients in senior living communities. You'll make home visits to evaluate functional status and develop individualized intervention plans. This role offers per diem flexibility and the chance to work with a 50-year industry leader.

🎯 What You'll Do

• Make home visits to clients in designated territories.
• Perform thorough evaluations of patients' functional status.
• Develop individualized intervention plans to achieve outcomes.
• Educate patients and family members on rehabilitative care.

📋 Requirements

• Current North Carolina Occupational Therapist license.
• Home health experience is required.
• Minimum one year of recent work as an Occupational Therapist.
• Graduation from an accredited occupational therapy program.
