You'll serve as the primary care provider for families in their homes, delivering comprehensive medical, behavioral, and social care. 🎯 What You'll Do

• Manage a panel of families as primary care provider in home and virtual settings.
• Collaborate with interdisciplinary team including MDs, social workers, and dieticians.
• Document member interactions and coordinate care using electronic health records.
• Attend daily huddles and weekly case conferences to establish health goals.
• Order diagnostic tests, prescribe medications, and educate patients on conditions.

📋 Requirements

• Active unrestricted Nurse Practitioner license in Arizona.
• 2 years experience providing clinical services to pediatric and adult patients.
• Proficiency in clinical nursing skills such as blood drawing and patient assessments.
• Availability to work full-time with rotating call including evenings and weekends.
