Description

You will make home visits to provide speech, language, and swallowing therapy for adult and geriatric patients. You'll develop treatment plans, educate patients and families, and document care using a touch pad tablet.

Requirements

• Current Speech Language Pathologist license in North Carolina
• Certificate of Clinical Competence in Speech from ASHA
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluation, physician's orders, and goals
• Educate and instruct patients and families on rehabilitative care and activities
• Accurately document patient care using a touch pad tablet
