Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, performing diagnostic tests, developing care plans, and educating clients and families to promote health and independent living.

Requirements

• Current NJ Physical Therapist license
• Minimum one year of recent work experience as a Physical Therapist
• Graduation from an APTA or COAE-approved program

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, range of motion, balance, etc.)
• Implement wound prevention and treatment interventions
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Document observations and interventions using a touch pad tablet
