💼 About This Role

You'll conduct home health admissions and provide skilled nursing care to patients in the Richmond area. You'll be part of a team that achieves industry-leading outcomes with low readmission rates.

🎯 What You'll Do

• Conduct Start of Care admissions and OASIS assessments
• Make home visits to patients in designated territory
• Administer IV medications and monitor patient conditions
• Document observations and collaborate with clinical team

📋 Requirements

• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid CA Driver's License
• Experience with OASIS and Start of Care

✨ Nice to Have

• Experience with high acuity patients
• Previous experience with IV medication infusions
• Case management fundamentals
