You'll evaluate and address patient needs in their homes, focusing on home safety, medication management, and falls prevention. As a member of the interdisciplinary care team, you'll serve as an educator and advocate to help patients live safely and productively. 🎯 What You'll Do

• Perform patient assessments and develop care plans.

📋 Requirements

• Current unrestricted CA Occupational Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
