Description

You will support the transition of infants, children, and adults from hospital to home care, partnering with CHOP to care for clients with rare and complex diseases. Your role includes delivering quality care and training the ongoing care team.

Requirements

• Current RN license
• NICU, PICU, ICU, or home care trach/vent experience
• Pass high-tech Adult and Pediatric simulations
• Current CPR certification
• Ability to commit to consistent schedule

Responsibilities

• Deliver quality care to clients transitioning from hospital to home
• Provide extensive training to ongoing care team
• Support infants, children, and adults with complex diseases
• Travel throughout Philadelphia County
