Description

You will provide 1:1 home health visits to adult and geriatric clients in Chester, York, and Lancaster counties, SC. You will follow care plans, administer medication, perform wound care, monitor conditions, and document care using a touch pad tablet.

Requirements

• Current Registered Nurse license in South Carolina
• Graduation from accredited nursing program
• Home health experience required if weekend-only

Responsibilities

• Follow designated care plan per patient's needs
• Make home visits in designated geographic territories
• Administer medication, wound care, treatments, procedures
• Monitor client conditions and report changes
• Accurately document observations and interventions
