Description

As a home care Physical Therapist, you will complete OASIS Start of Care visits for adult and geriatric patients in Essex County, developing and implementing individualized care plans to promote health, safety, and independent living while accurately documenting patient care using a touch pad tablet.

Requirements

• Current New Jersey Physical Therapist License
• Minimum one year recent PT work experience
• OASIS experience required
• HCHB preferred

Responsibilities

• Make home visits for Start of Care admissions
• Perform diagnostic tests and measurements
• Develop and implement individualized care plans
• Educate clients and family on rehabilitative care
• Document observations and interventions using a tablet
