Description

You will perform home health visits for adult and geriatric patients in Addison County, Vermont, as part of a multi-disciplinary team. You will assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties, developing treatment plans and educating patients and families.

Requirements

• Current license as a Speech Language Pathologist in Vermont
• Certificate of clinical competence in speech from ASHA or equivalent educational/work experience
• Solid computer skills; prior experience with electronic medical records preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations to assess and diagnose speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluations and physician's orders
• Educate and instruct patients and family members in rehabilitative care
• Document observations, interventions, and evaluations using a touch pad tablet
