Description

You will provide skilled physical therapy to adult and geriatric patients in their homes, helping them maintain independence through individualized care plans and rehabilitative interventions.

Requirements

• Current Physical Therapist license in Massachusetts
• Prior home health experience strongly preferred
• Ability to provide care to adult and geriatric patients

Responsibilities

• Make visits to clients in their homes
• Perform diagnostic tests and measurements (mobility, range of motion, balance, etc.)
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and family members on rehabilitative care
• Document observations and interventions using a tablet
