Description

You will provide psychosocial services to terminally ill patients and their families, including assessments, counseling, and community resource referrals. You will collaborate with an interdisciplinary team to develop and coordinate care plans.

Requirements

• Master's degree in social work from a CSWE-accredited program
• Valid Ohio driver's license and insured reliable transportation
• Active Ohio Social Work license
• Minimum 1 year health care experience with terminally ill patients and families
• Hospice or home care experience preferred

Responsibilities

• Conduct initial psychosocial and bereavement risk assessments within 5 days of patient election of hospice care
• Facilitate referrals to community resources, including financial assistance
• Provide psychosocial and anticipatory grief counseling to patients and families
• Participate in interdisciplinary team meetings to coordinate care plans
• Prepare social histories, evaluations, and clinical documentation
