💼 About This Role

You'll provide skilled nursing and rehabilitative care to adult clients in their homes, as part of a multi-disciplinary team. Your core impact is enabling clients to receive medical care while remaining at home. One year prior clinical LPN experience and a current VA license are required.

• Making home visits to clients in designated territories
• Administering medication, wound care, treatments, and procedures
• Monitoring clients' conditions and reporting changes
• Documenting care in electronic medical records

📋 Requirements

• Current LPN license in Virginia
• Minimum one year recent clinical LPN experience
• Graduation from accredited nursing program
