Description

You will be an integral member of a multi-disciplinary team providing skilled nursing and palliative care to clients at home. You'll conduct clinical social evaluations, coordinate community resources, and advocate for clients and families to navigate the healthcare system.

Requirements

• Licensed by NJ Board of Social Work Examiners at LMSW or LISW level
• Master's in Social Work with at least 2 years community-based experience
• Accreditation by Council on Social Work Education
• Ability to travel to cases as assigned
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated territories
• Provide clinical social evaluations and plan appropriate interventions
• Identify community resource referrals for client and family needs
• Participate in development of total plan of care with multidisciplinary team
• Educate and instruct patients and families in community resources
