Description

You will provide 1:1 skilled nursing care to clients in their homes, performing assessments, administering medications, wound care, and patient education. This role offers flexible daytime schedules and focuses on delivering compassionate, high-quality care in a small geographical territory.

Requirements

• Current MA nursing license in good standing
• At least one year of recent acute clinical experience as an RN
• Med/surg experience strongly preferred
• Previous home healthcare experience strongly preferred
• OASIS documentation experience strongly preferred

Responsibilities

• Perform 5-6+ visits per day in a small geographical territory
• Deliver 1:1 skilled nursing including medication administration, wound care, and treatments
• Perform client assessments and monitor patients' conditions
• Provide education to patients and their families
• Implement individualized care plans
