Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, developing and implementing individualized care plans, performing diagnostic tests and measurements, and educating clients and families to promote health and independent living.

Requirements

• Current Maryland Physical Therapist license
• Minimum one year of recent work experience as a Physical Therapist
• Graduation from a program approved by the American Physical Therapy Association
• Strong interpersonal, organizational, and problem solving skills
• Solid computer skills; prior experience with electronic medical records preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements such as mobility, range of motion, and balance testing
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate and instruct clients and family members in rehabilitative care activities
• Document observations, interventions, and evaluations using a touch pad tablet
