💼 About This Role

You'll perform home health visits for adult and geriatric patients in Baltimore County as part of a multi-disciplinary team. You'll evaluate functional status, develop care plans, and educate patients and families to promote independence. This role offers the chance to make a meaningful impact in patients' homes.

🎯 What You'll Do

• Make home visits to clients in designated territories
• Perform thorough evaluations of functional status
• Develop and revise individualized intervention plans
• Educate patients and families on rehabilitative care

📋 Requirements

• Current Maryland Occupational Therapist license
• Minimum 1 year of recent experience as an OT
• Graduation from an accredited occupational therapy program
