💼 About This Role

You'll serve as the face of FreedomCare in Oregon, performing initial monitoring and quarterly assessments of patients on care. You'll also train caregivers to ensure they provide the best service possible to their loved ones.

🎯 What You'll Do

• Conduct home orientations and sign paperwork
• Perform initial in-home assessments
• Conduct periodic supervisory visits for on-care cases
• Train caregivers and patients as required per case

📋 Requirements

• 2+ years experience with frail or elderly disabled adults
• Previous field experience in medical or case management
• Valid driver's license with insured vehicle
• Proficient in Microsoft Office (Word, Excel, Outlook)

✨ Nice to Have

• Experience with Salesforce
• Hands-on home care experience
• Working knowledge of ADL/IADL assessments
