You will provide 1:1 home health visits to clients in Orleans County, VT, following a designated care plan. Responsibilities include medication administration, wound care, treatments, monitoring client conditions, and documenting care using an electronic tablet. Requirements

• Active RN license in Vermont
• Minimum 1 year recent clinical experience
• Graduation from accredited nursing program
• Ability to work independently and travel to assignments
• Strong interpersonal and computer skills

Responsibilities

• Follow designated care plans and make home visits
• Administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes
• Perform client assessments and case management
• Accurately document observations and interventions using EMR
