💼 About This Role

You'll provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes as part of a multi-disciplinary team. Your core impact is helping clients regain speech, language, communication, or swallowing abilities so they can remain at home. You'll also document care using a state-of-the-art touch pad tablet.

🎯 What You'll Do

• Perform home visits to clients in designated geographic territories.
• Conduct evaluations to assess and diagnose communication or swallowing difficulties.
• Develop and revise treatment plans based on evaluation and physician orders.
• Educate patients and families on rehabilitative care and activities.

📋 Requirements

• Current Speech Language Pathologist license in Pennsylvania.
• Certificate of Clinical Competence in Speech from ASHA or equivalent educational requirements.
• Solid computer skills.
• Must be able to complete supervised work experience for certification if not yet obtained.
