You'll oversee comprehensive care coordination for patients in a tech-powered family care platform. Your work directly supports family caregivers, improving health outcomes and experiences.

🎯 What You'll Do

• Manage a diverse caseload ensuring quality care and coordination.
• Provide coaching and oversight to Certified Nursing Assistants.
• Develop and implement individualized care plans.
• Coordinate with PCPs, therapy providers, and internal teams.

📋 Requirements

• Associate's or Baccalaureate degree in Nursing.
• Registered Nurse license in PA or Compact/Multi-state.
• 2 years experience as RN in clinical setting.

✨ Nice to Have

• 1 year of home health experience.
• Case management or supervisor experience.
• Knowledge of Medicaid paid family caregiving program.
