You'll join Pair Team's interdisciplinary care model to support individuals with complex needs, using evidence-based approaches to promote engagement and health goal achievement. You'll help build trust-based relationships with underserved populations, ensuring they receive whole-person care that reduces hospital visits.

🎯 What You'll Do

• Maintain ongoing caseload using evidence-based approaches to promote engagement
• Conduct periodic telephonic and SMS outreach for timely follow-up
• Identify health/wellness goals and incorporate into Health Action Plan
• Coordinate appointments and manage referrals to community supports

📋 Requirements

• 1+ years of general work experience (case management preferred)
• Physically located in the United States
• Bilingual – English/Spanish
• Valid driver's license and auto liability insurance

✨ Nice to Have

• Living experience with individuals with complex chronic needs or SMI/SUD
• 1+ years of case management experience
• Familiarity with motivational interviewing and medical terminology
