💼 About This Role

You'll serve as the first point of contact for patients, educating them about Upward Health's in-home, multidisciplinary care. You'll manage a caseload primarily through phone outreach and help enroll individuals into whole-person care programs. This role is essential for connecting patients to services that treat physical, behavioral, and social health needs.

• Educate patients about Upward Health's services and benefits
• Engage potential patients via phone, in-person visits, and mailings
• Facilitate patient enrollment into programs and manage caseload
• Collaborate with outreach team to achieve engagement goals

📋 Requirements

• Strong verbal communication and persuasive abilities
• Proficiency in using computer systems for documentation and outreach
• Self-motivated with ability to work independently and meet goals
• Fluent in English (Spanish is a plus)
