Description

You will provide compassionate, 1:1 rehabilitative care to adult patients in their homes, helping them recover from injury, illness, surgery, or hospitalization. You'll enjoy flexible scheduling, hands-on training, and 24/7 clinical support as part of an interdisciplinary team.

Requirements

• Active and current Physical Therapist license in Ohio
• Graduation from an APTA- or CAAHEP-approved program
• Ability to travel locally to cases
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform diagnostic tests and measurements (mobility, range of joint motion, transfer status, balance, safety assessments)
• Implement wound prevention and repair interventions using evidence-based dressings and topical agents
• Develop and implement individualized care plans including manual therapy, gait training, and balance exercises
• Educate clients and families on rehabilitative care and activities for health, safety, and independent living
