💼 About This Role

You'll manage direct nursing care for participants in a PACE program, acting as the quarterback for your panel within an interdisciplinary team. You'll drive personalized care plans and coordinate complex transitions of care across clinic, home, and SNF settings. This role offers the chance to redefine aging in place with a supportive team culture.

🎯 What You'll Do

• Conduct face-to-face nursing assessments in clinic and home settings.
• Deliver and document nursing interventions per participant care plans.
• Coordinate complex patient discharges and transitions of care.
• Triage in outpatient setting and initiate therapies independently.

📋 Requirements

• 2-4 years of clinical RN experience with older adults or medically complex population.
• 1-2 years of case management experience.
• Unencumbered Registered Nurse (RN) license in California.
• Experience with wound care, IVs, and phlebotomy.

✨ Nice to Have

• Bachelor of Science in Nursing (BSN).
• Alzheimer’s certification.
• Bilingual Spanish.
