💼 About This Role

You'll conduct in-home health risk assessments for patients across New Jersey as part of Porter's expanding healthcare team. Your assessments will drive personalized care plans that improve outcomes and reduce costs.

🎯 What You'll Do

• Visit members in assigned geographic area for in-home assessments
• Perform health risk assessments including medical history and physical exam
• Conduct point-of-care testing as appropriate
• Formulate list of diagnoses for care management

📋 Requirements

• Valid unencumbered New Jersey NP license
• 3 years of patient care experience
• EMR skills preferred
