Description

You will provide after-hours hospice care to patients and families, conducting assessments, managing symptoms, and coordinating care during evening and weekend shifts. This role involves a rotating schedule: one week on (Monday through Thursday overnight and continuous Friday through Monday) followed by one week off.

Requirements

• Bachelor's or Associate's degree in nursing or related field
• Active nursing license in good standing in Indiana
• Active CPR certification
• 2-5 years experience in healthcare management, hospice, or palliative care preferred
• 1 year experience with Medicare Hospice Certification and CHAP guidelines preferred

Responsibilities

• Complete initial and comprehensive patient assessments for hospice needs
• Provide professional nursing care using nursing process and health assessment data
• Develop and revise care plans with patient and family involvement
• Administer medications and treatments as prescribed
• Communicate with physicians and coordinate care after hours
