🎯 What You'll Do

• Conduct one-on-one coaching sessions in homes or community settings
• Educate patients about chronic conditions like diabetes and COPD
• Help patients engage with primary care and schedule visits
• Document patient encounters in Salesforce

📋 Requirements

• 2+ years in customer service, community work, or case management
• High School diploma or GED required
• Strong interpersonal and organizational skills
• Comfort with technology and systems like Salesforce

• Some college or healthcare experience
• Experience with Motivational Interviewing
• Care coordination skills
