Description

As a Medical Social Worker, you will be an integral member of a multi-disciplinary health care team providing skilled nursing and rehabilitative care to clients in their homes, helping them remain at home. You will conduct clinical social evaluations, identify community resources, and advocate for clients and families.

Requirements

• Master's Degree in Social Work
• Licensed Social Worker (LCSW or LICSW) in Massachusetts
• Prior home care experience preferred but not required

Responsibilities

• Make home visits to clients in designated geographic territories
• Provide clinical social evaluations and plan appropriate interventions
• Identify community resource referrals for client and family needs
• Act as advocate for client and family to navigate community system
• Participate in development of total plan of care with multidisciplinary team
