🎯 What You'll Do

• Manage performance and development of assigned Health Coaches
• Lead weekly team meetings to discuss successes and challenges
• Shadow Health Coaches and provide feedback on patient engagement
• Monitor performance dashboards and facilitate feedback sessions

📋 Requirements

• 3+ years of supervisory experience
• Experience navigating social determinants of health
• Strong patient-facing or customer service experience
• Ability to drive on-site for meetings and patient visits
