💼 About This Role

You'll guide participant care as a key member of the interdisciplinary team at WelbeHealth PACE. You'll conduct assessments, develop care plans, and coordinate transitions for seniors. This role offers a consistent Monday-Friday schedule with no weekends or on-call.

🎯 What You'll Do

• Conduct clinic nursing assessments to determine participant needs and goals.
• Deliver and document nursing interventions per care plans.
• Collaborate with physician and IDT to coordinate participant care.
• Instruct participants and families on disease process and self-care.

📋 Requirements

• Graduate of an accredited school of nursing.
• Unencumbered California RN License.
• Current Basic Life Support (BLS)/First Aide Certification.
• Two years of nursing experience in a clinical setting with frail or elderly population preferred.
