Description

You will provide rehabilitative speech therapy to adult and geriatric patients in their homes, conducting evaluations and developing treatment plans as part of a multi-disciplinary team. This role allows patients to receive essential medical care at home.

Requirements

• Current Speech Language Pathologist license in VA
• Certificate of Clinical Competence in Speech from ASHA or equivalent educational and work experience
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform evaluations and provide therapeutic interventions for speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluation and physician's orders
• Educate patients and families on rehabilitative care for independent living
• Document observations and interventions using electronic medical records
