Description

You will provide comprehensive psychosocial services to terminally ill patients and their families, including assessments, counseling, and care coordination. You'll work collaboratively with an interdisciplinary team to develop care plans that respect patient choices and ensure compassionate support.

Requirements

• Master's degree in social work from a CSWE-accredited school
• Active Ohio Social Work license
• Valid driver's license with insured reliable transportation in Ohio
• Minimum 1 year healthcare experience with terminally ill patients and families
• Hospice or home care experience preferred

Responsibilities

• Conduct comprehensive psychosocial and bereavement risk assessments within 5 calendar days of hospice election
• Facilitate referrals to community resources, including financial assistance and placement in facilities
• Provide psychosocial and anticipatory grief counseling to patients, families, and caregivers
• Participate in interdisciplinary team meetings to coordinate care and address social/emotional factors
• Maintain accurate clinical documentation and assist with discharge planning
