Description

As a Medical Social Worker on our home health team, you will provide clinical social evaluations, connect clients and families with community resources, and advocate for their needs while collaborating with a multidisciplinary team. You'll use a touch pad tablet for documentation and travel to clients in the Seacoast New Hampshire area.

Requirements

• Master's in Social Work from CSWE-accredited program
• Minimum 2 years community-based experience
• Ability to travel to client homes
• Strong interpersonal and communication skills
• Computer proficiency, EMR experience preferred

Responsibilities

• Conduct clinical social evaluations and plan appropriate interventions
• Identify and refer clients to community resources
• Advocate for clients and families navigating the healthcare system
• Collaborate with multidisciplinary team to develop total plan of care
• Document observations and interventions using electronic tablet
