Description

As a home care Physical Therapist, you will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, enabling them to remain independent. You'll develop individualized care plans, perform diagnostic tests, and educate clients and families.

Requirements

• Current New Hampshire PT license
• Minimum 1 year recent clinical experience as a Physical Therapist
• Graduation from an accredited PT program
• Ability to work independently
• Solid computer skills; EMR experience preferred

Responsibilities

• Conduct home visits to clients in assigned territories
• Perform diagnostic tests and assessments (mobility, strength, balance, etc.)
• Develop and implement individualized care plans with therapeutic exercises
• Educate clients and families on rehabilitative care and safety
• Document observations and interventions using electronic medical records
