💼 About This Role

You'll provide after-hours hospice care to patients in their homes, performing assessments and managing symptoms. You will coordinate with the interdisciplinary team to ensure compassionate end-of-life support. This role offers a unique rotating schedule with one week on, one week off.

🎯 What You'll Do

• Complete initial and comprehensive patient assessments for hospice needs.
• Provide professional nursing care and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Coordinate patient care after hours and support case managers.

📋 Requirements

• Bachelor's or Associate's degree in nursing or related field.
• Active nursing license in good standing in Texas.
• Active CPR certification.
• 2-5 years of experience in hospice, palliative care, or healthcare management.

✨ Nice to Have

• 1+ year Medicare Hospice Certification and CHAP guidelines experience.
• Knowledge of hospice services and interdisciplinary group care.
