Join K Health to deliver fully remote, video-based primary care including preventive, acute, and chronic care, supported by a cutting-edge clinical AI platform. You will manage a dedicated patient panel and contribute to population health initiatives.

Requirements

• Board-Certified in Family Medicine
• 3+ years post-residency work experience
• 2+ years in an in-person primary care setting
• Experience with virtual visits required
• Active MA state medical license, IMLC preferred

Responsibilities

• Provide high-quality preventative, acute, and chronic care in a fully-remote virtual setting
• Manage a dedicated panel of patients as their primary care physician
• Refer patients to specialists or acute care services when clinically indicated
• Work on population health initiatives for risk-attributed patients (AWVs, care gaps, transitions of care, risk capture)
• Support patients post-discharge to improve treatment plan understanding and reduce readmissions
